RG500 not booting up correctly after firmware update

Hi, I have a 5G modem uses this firmware version
RG500QEAAAR11A03M4G_ACY

But after the upgrade, the modem doesn’t boot back to normal mode.
It shows Limited LTE state. Qualcomm/Diagnostic
With a USB Host3-Port1 Device(0x05c6/0x9008/1) plug-in detected

which I think is the Qualcomm Diagnostic Port.

How can I revive this ?
Can I please get a latest copy of the firmware RG500QEAAAR11A03M4G_ACY
as well as the QFlash ?

This firmware was pushed by the carrier - Telstra in Australia
Modem is now half-dead only GUI working, the modem module is stuck in Qualcomm diag mode
