According to BCM47755B1 information, chip only has 515 m/s speed limit - but LC79D module datasheet has old 18000m altitude & 515 m/s speed limitations listed. Is datasheet incorrect or does Quectel apply additional limitations to PVT output?

Yes. It is additional limitation applied by Quectel. Because we found that if module exceed altitude or speed limits, it will work abnormally, like get fixed for a few seconds and get lost for a few seconds.
